Data Entry and Management

Actuarial Work-Products, Inc. offers the user these methods of production:

Method 1
The User will provide the requisite data to Self-Funding Actuarial Services, Inc in the usual unformatted and unstructured manner and will be provided a hard-copy actuarial work-product in the usual manner.

Method 2
The User will provide the requisite data to the Actuarial Work-Products, Inc. in a formatted and structured manner and will be provided a hard copy actuarial work-product prepared in the usual manner. Please see the Data Transmittal Memorandum (DTM) on the previous page for a copy of the hard-copy form.

Method 3
The User will, if approved, gain Eligible User status with Actuarial Work-Products, Inc. Such Eligible User will input the requisite data directly into such dedicated Website that contains an embedded computer program. An actuarial work-product, with an appropriate signature, will be e-mailed to the Eligible User and other addressees as requested.
